We ask that first time patients and current patients whose insurance has changed to please bring your insurance card, and if available, the policy booklet.
Please remember that no insurance company attempts to cover all dental costs. Some companies pay fixed allowances for certain procedures and others pay a percentage of the charge. It is your responsibility to pay any deductible amount, co-insurance, or any other balance not paid for by your insurance company.
We know questions can arise on insurance matters. We encourage you to discuss such questions with our business office staff. We will be happy to help you receive the maximum benefits; however, the agreement of the insurance company to pay for your dental care is a contract between you and the insurance company.
Your appointed time is just for you. We kindly request a 48 hour notice should you need to reschedule.
Help us serve you more efficiently by filling out this downloadable 3-page PDF form and bringing it along with you when you visit our office. The more we know about you and your medical / dental history, the better we can take care of you.
If you’ve recently had a dental operation performed by Dr. Neal, such as a root canal or a composite restoration, and are having any issues, please feel free to download and read these information sheets about proper care following procedures.
